You can fill in your medical information here and it will generate a QR code and provide you with the link to view your information, you may then save the QR code and print it and store it somewhere like your wallet or a sticker on your helmet. It's highly recommended that you keep it on your body and not your bike as you may not be with your bike if you crash.
If you wish to edit your medical information please contact me at brandon@lostinmotion.store and I will update it, same if you wish for it to be removed.
